I HEREBY CONSENT to the student’s use of the training facilities and participation in the martial arts styles offered and in turn agree to the terms and conditions

below which I have read and understood prior to the student signing this agreement and my signing of this consent and discharge form. I AGREE to be liable for and

INDEMNIFY the Organisation, instructor and fellow students against any damage (as defined within) suffered or incurred by the Organisation, instructors or fellow

students as a result of any action or failure to act, and any behaviour whether negligent or otherwise of the Student and whether caused by the Students breach of

any term and conditions above appearing or the rules and regulations of the Organisation, instructor or fellow students.

1. In CONSIDERATION of the Organisation permitting me to train and to study in the martial art style I agree as follows: I understand that participating in a martial art style is DANGEROUS and I voluntarily ACCEPT the risk of damage consequent upon or arising from my participation as a student or instructor, and the use of the organisations or instructors facilities.

2. I WILL NOT sue the Organisation or the Instructors for any negligence, tort, breach of contractual or any other legal or equitable rights howsoever caused, and this indemnity will extend to and include any damage arising from training, demonstrating and/or grading in the martial arts style and from my use of the Organiser or instructors facilities and I INDEMNIFY the Organiser or Instructors in respect of the same.

3. I WILL abide by the rules and regulations of the Organiser or instructor’s as to the practices of the martial arts style and to the use of the Organisers or instructor’s facilities and the directions of the Organiser or instructor’s officials including the right to terminate or cancel my martial arts training or grading and the use of the Organiser or Instructor’s facilities at any time and for any reason.

4. THE PERSONAL INFORMATION I have supplied to the Organisation or instructor regarding my qualifications, experience, medical conditions and any other matter associated with the martial arts style is true and correct and I have read and understood all of the clauses of this agreement before signing the same and before my use of the Organiser or Instructor’s facilities or before any grading or demonstration. I agree to notify the Organiser or instructor in writing of my health problems relevant to any such training, study, demonstration or grading.

5. I AGREE to any first aid treatment to be given to me by the Organiser or any of our Instructors if they feel it to be necessary mean:

Organiser or Instructors – Fortress Jiu Jitsu and its Owner, Directors, Shareholders, all instructors, trainee instructors, fellow students, officers, mangers, advisors, employees, agents, licensees, sub-contractors, subsidiaries, holding company’s, associates and assignees or any person or company associated with the company in any way.

The Organiser or instructors facilities – including the land and buildings and equipment associated with any martial arts training, demonstration, grading, accommodation, and function or training venue.

Use of the Organiser or instructor’s facilities – the use by the student or his attempted use or the Organiser or instructors facilities whether such use or access is in breach of this agreement or the Organiser or instructor’s Rules and Regulations or authorised or otherwise and whether intended to be used or not.

Damage – all loss or damage, costs or expenses, whether direct or indirect flowing from any legal liability, claim, demand right of action, proceedings or judgements of whatever nature and whether arising at law or in equity and whether suffered to the person or property of the Organiser or instructor, the student or otherwise.

Rules and regulations - the Rules and Regulations are the Rules and Regulations relating to any martial arts style including verbal Rules and Regulations which are available from the Organiser or instructor, and include all amendments or alterations to the Rules and Regulations made from time to time.

Membership Terms and Conditions:

1. Our academy provides weekly grappling classes for adults and juniors*, encompassing Gi and no-Gi formats.

2. The curriculum covers essential aspects, including control, takedowns, defence, and submissions, ensuring a thorough learning experience for members.

3. All members must adhere to the academy's rules and guidelines to maintain a safe and respectful training environment.

4. Participation in classes implies acceptance of these Terms and Conditions, with any violation subject to disciplinary action at the discretion of the academy management.
"5. Membership fees are non-refundable and non-transferable.

6. Members are required to wear appropriate attire and adhere to hygiene standards during all training sessions.

7. The academy reserves the right to modify class schedules, instructors, and facility access without prior notice.

8. Any damages to academy property caused by members must be reimbursed promptly.

9. Members are responsible for their personal belongings; the academy is not liable for lost or stolen items.

10. The academy may terminate or suspend membership privileges for misconduct or violation of these Terms and Conditions.

11. By signing up for membership, individuals agree to receive communications from the academy regarding schedule changes, promotions, and other relevant updates.

12. These Terms and Conditions are subject to change at the discretion of the academy management."

"Minimum Membership Contract Length:

Memberships entail a minimum contract length, as specified upon enrollment.

Cancellation Policy for Direct Debits:

Memberships and direct debits can be cancelled at any time. It is the member's responsibility to cancel their direct debit with their bank. Upon cancellation of the direct debit, access to Fortress Jiu-Jitsu Academy will continue until the next payment date. Following this, access will be terminated without refunds for any remaining days of membership. For instance, if a direct debit payment is made on the 1st of March and cancelled on the 14th of March, access will be valid until the 1st of April, with no refunds offered between the 15th and the 31st of March.

Payment Methods:

Payments are processed via direct debit on the 1st of each month. Members can review and update payment details through the member's portal.

Membership Cancellation Process:

Minimum Notice: 1 month.

Rejoining Later: If you cancel and decide to rejoin later, a £10 administration fee will be applied.

Freezing Your Membership

Duration: You can freeze your membership for 1-3 months.

Freeze Fee: A £10 administration fee applies.

Reactivation: If the membership is not reactivated within 3 months, it will be cancelled, and you will need to pay a sign-up fee to rejoin.

Cancellation Policy for Single Session or Day Pass:

We understand that plans can change unexpectedly, and we strive to accommodate our members' needs to the best of our ability. However, to maintain fairness and operational efficiency, we have implemented the following cancellation policy for single-session or day pass bookings:

1. Cancellations must be made at least 24 hours before the scheduled session or day pass usage.

2. Failure to provide at least 24 hours' notice of cancellation may result in forfeiture of the session or day pass without a refund.

3. Cancellations made within the specified timeframe will be eligible for a refund or rescheduling of the session or day pass, subject to availability.

4. Refunds for cancellations made within the specified timeframe will be processed within [insert timeframe, e.g., 5-7 business days] to the original form of payment.

5. In the event of unforeseen circumstances or emergencies, exceptions to this policy may be granted at the discretion of Fortress Jiu Jitsu.
